[Stk] STK version 4.0, official release

The new release of the Synthesis ToolKit in C++, version 4.0, is 
finally ready.  For complete information, surf to:

  http://www-ccrma.stanford.edu/software/stk/

What's new:

- complete documentation (PDF manual and HTML)!
- new instruments (yea!) 
- lots of API changes (boo)

Unfortunately, we can pretty much guarantee that you will have to make 
some changes to your old classes in order for them to compile with the 
new release.  The good news is that the changes are for the better and 
you now have code documentation (thanks to doxygen) to help during the
conversion process.
