What you describe seems bad; first a different card has trouble on the MAC, then your card comes back broken.

To start, I'd check the the output jacks/connectors and look for any damage/loose stuff.

Then, I'd connect the bad outputs to something with high gain/low noise and listen to the weak outputs.
If there's distortion it may suggest that the outputs are blown, which would be too bad.

I don't know anything about the card, like if it has some internal firmware the has been set by the MAC, but I doubt it.
But you should check it out just in case.

Anyway, I hope you get it working.
