[CM] new chapters available

Ive made the entire second half of the book available again. this link 
should always work, just click on the glowing lambda...

the material should now have a 100% consistent presentaton style and 
ive slightly improved the 'navbar' thanks to tobias. ive only tested 
the examples in openmcl/osx -- ill try them out in guile and cmu over 
the course of the next day or so and then update the cm tarball with 
the fixes, including (hopefully) a dispatch macro that works everywhere.
