It should be applied at the top level.  It patches several files.

$ gunzip stk-4.1.1.patch.gz
$ tar zxf stk-4.1.1.tar.gz
$ cd stk-4.1.1
$ patch -p 0 < ../stk-4.1.1.patch

I don't know how to make rpms :/
