[CM] old cmdist archives

The old cmdist archives (files with names like cmdistMail94.gz
on ccrma-ftp) have been replaced by old-cmdist.html.  I removed
most (16000 lines) of the mailer blathering, added a table of
contents, and prettified it somewhat.  Take a stroll down memory
lane.

(By the way, by my count I made about 4 more posts than Rick over
the course of 8 years -- must remember to mention this in my
next performance review).
