new implementation of defscale

ive reimplemented defscale to allow the creation of gapped and general scales in addition to equal tempered.  its also now more flexible and easier to use than the original version and lets you layout a scale's frequencies three different ways: directly, as pitch values, or indirectly as scalers or interval distances between degrees.  ive made a patch file:
	ftp.zkm.de:/pub/defscale.tar.Z
that includes three files: a new version of scales.lisp, a new file contrib/defscales.lisp containing a bunch of different types of scales (ie flavors of javanese, bohlen-pierce, pythagorian, stretched octave, microtone etc etc etc), and a new doc/dictionary.rtf with updated entries for defscale and news ones for cent, centify, find-scale and list-all-scales
